My authorization to contribute monthly from the account I’ve indicated shall remain in effect until I notify OPB that I wish to change or end it, and OPB has had reasonable time to act on my wishes. I understand that my updated contributions will begin in one to five days. If my monthly contribution has already processed, the change will occur in one month. Contributions will occur monthly, on or near the same day of the month, thereafter. A record of each contribution will appear on my monthly checking account or credit/debit card statement and will serve as my receipt. You must agree to these terms to continue.
